Fill in the details of your foal(s) and the parents. You’ll get a ready-to-use Zoolyx form as a PDF to send along with the hair samples.
Send the hair with roots, each horse in a separate sealed bag labelled with the animal’s name, to Zoolyx, Groeneweg 17, B-9320 Aalst, Belgium. Already have a DNA card for the dam and/or sire? Include it (their hair is then not needed). Email a copy of this document to info@eafs.be.
